Patient's Profile

Ms LC, in her late 40s, had felt self-conscious about the fullness under her chin in recent years.

Background

Ms LC has always been careful with her diet. She has made proactive efforts, including eating less, reducing her carbohydrate intake, and exercising regularly — especially during the Covid period — to maintain a healthy lifestyle. 

"I've always taken care of myself, but my double chin made me look heavier and older than I felt. Trust me, I tried everything from massages to gua sha and fat freezing, but nothing worked," she admitted. "It was frustrating because I felt I had no control over it. It affected my confidence, especially in photos and video calls." 

After over a dozen sessions of fat freezing without any improvement, Ms LC sought a second opinion from Dr Ivan Puah.

Assessment and Consultation

During the consultation, Dr Ivan Puah listened carefully to her concerns and assessed her clinical condition.

A detailed examination revealed that Ms LC has a moderate degree of loose skin, saggy jowls, and localised submental fat, commonly known as a double chin.

He explained that genetics and ageing often contribute to stubborn fat in the chin and jawline. 

Unfortunately, the fat accumulated in this area typically does not respond to dieting or exercising.

Non-surgical methods like fat freezing may or may not improve the condition. In her case, fat freezing might not yield positive results due to her submental fat and loose skin.

He recommended facial liposuction, a minimally invasive procedure that would remove unwanted fat from the chin, enhancing her natural facial contours and sculpting her jawline.

Ms LC would also benefit from Ultherapy, a non-surgical skin tightening treatment designed to firm and lift loose skin in the jowls and neck area.

After carefully considering the pros and cons, she decided to proceed with the surgery. A series of blood tests were conducted to ensure she was fit for the procedure.

Treatment Plan

On the day of the surgery, the treatment area was marked, and facial liposuction was performed under local anaesthesia while the patient was lightly sedated.

Throughout the one-hour procedure, the anesthesiologist closely monitored her vital signs.

Dr Puah made several small incisions to allow the infiltration of anaesthetic fluid into the targeted facial area.

Then, a specialised thin and blunt liposuction cannula was inserted to gently suction out excess fat, sculpting a more defined jawline.

The incisions were then closed, and compression dressings were applied. Ms LC recovered well during the day of the surgery and was discharged home the same day with detailed post-operative instructions.

Post-Op Recovery & Results 

Ms LC had her stitches removed ten days after the surgery. 

Initially, she experienced some swelling and bruising, which made her worried.

However, she diligently wore the facial compression garment and realized that it helped her recovery.

The post-operative discomfort was less than she had expected. Dr Puah advised her to keep the compression garment on for eight weeks for optimal results.

During her last review, Ms LC saw her "before" and "after" pictures, which showed a significant improvement. She was very satisfied with the results. 

Subsequently, she decided to proceed with Ultherapy, aiming for firmer, tighter facial skin to lift her brows and counteract signs of ageing.
