Patient's Profile
Mr G.E, in his 30s, had been struggling with bilateral gynecomastia (enlarged male breasts) for years.
Despite losing over 50kg during his National Service (NS), his chest appeared more prominent due to moderate skin laxity.
He had tried steroid injections previously.
As a client-facing executive who frequently wears fitted shirts, the visible chest caused him considerable distress and self-consciousness.
In 2025, Mr G.E scheduled a consultation at Amaris B. Clinic.
Consultation & Assessment
Though he achieved substantial weight loss during NS, Mr G.E's chest contour remained problematic, further highlighted by sagging skin.
He rated the emotional and physical impact of his condition as 9 out of 10.
Despite years of weight management and exercise, the stubborn chest fullness persisted.
This affected not only his appearance but also his confidence, especially in formal and social settings where he felt unable to dress comfortably.
During the consultation, Dr Ivan Puah diagnosed Mr G.E with Grade III gynecomastia with moderate skin laxity (Type 4/5).
Excess glandular tissue and fat had obscured the natural pectoral border, giving his chest a rounded, feminine appearance.
Due to skin laxity, his nipples had descended below the infra-mammary fold, creating an undesirable chest contour.
Dr Puah explained that gynecomastia surgery would be the most effective permanent solution.
The goals were to:
Remove excess breast tissue and fat
Tighten loose skin
Resculpt a masculine, athletic chest contour
Special emphasis was placed on achieving a natural, masculine chest (flattened without over-resection) to avoid an unnatural "overdone" look.
The aim was a proportionate, V-shaped torso that would allow Mr G.E to wear fitted clothing with confidence.
After thorough discussion and much consideration, including potential risks, side effects, and expected outcomes, Mr G.E decided to proceed.
Pre-operative blood tests were conducted to ensure fitness for surgery.
Treatment Plan
The procedure was performed under twilight sedation at an accredited Day Surgery Centre, with continuous monitoring of vital signs by the anaesthetist throughout.
The incision was discreetly placed along the lower alar border for minimal visibility.
Excess chest fat was removed using the MDC-Sculpt® Lipo Technique, which contours the chest while preserving natural masculine topography.
Together, these techniques ensure comprehensive removal of tissue and fat, enhanced chest definition, and minimal scarring, making them ideal for lasting gynecomastia correction.
Post-Op Recovery & Results
Mr G.E experienced minimal bruising and soreness after surgery.
Stitches were removed after 10 days, and he was advised to wear a compression vest continuously for 8 weeks to support skin contraction and healing.
Results were visible immediately post-op, with continued improvement over the following 3 months as swelling subsided.
Mr G.E diligently applied scar gel and sunscreen to the incision sites whenever they were exposed to sunlight. Over time, the scars healed well and became barely noticeable.
Today, the patient enjoys a flatter, firmer, and more masculine chest; free from the self-consciousness that once affected his personal and professional life.
A Note from Dr Ivan Puah
"Mr G.E's case presented distinct surgical complexities due to the exceptionally large and fibrous glandular tissue. Cases like his require not only precise technique but also considerable experience to ensure complete gland removal while minimising risks such as haematoma.
The procedure took longer than usual because meticulous attention was paid to thoroughly dissecting the glandular tissue and ensuring optimal contouring. This level of care is essential to achieve a smooth, natural chest contour and reduce post-operative complications; outcomes that come with surgical experience and a deep understanding of male chest anatomy."
SCARLESS GYNECOMASTIA SURGERY WITH 360°GTD® TECHNIQUE
