What Is Gynecomastia?

Gynecomastia (colloquially "man boobs") can occur in one or both breasts, often with tenderness around the breast and nipple.

"Gynecomastia, or the development of excess glandular tissues, can occur when there is a hormonal imbalance in men," says Dr Ivan Puah, medical director of Amaris B. Clinic.

Other causes include ageing and medication side effects.

Causes of Gynecomastia

Gynecomastia has four root causes:

Physiological

Patients may develop physiological gynecomastia due to hormonal imbalance in young adults, adolescents, or the elderly.

Pharmacological

Gynecomastia can be due to pharmacological causes, such as the use of anabolic steroids, or the long term consumption of certain types of medications such as anti-depressants. 

Pathological

Very rarely, certain hormone-producing tumours, such as testicular or brain tumours, may lead to the development of gynecomastia. The underlying cause must be addressed to treat this group of patients.

Idiopathic

Gynecomastia is most frequently caused by idiopathic conditions without an established aetiology.

Physical & Psychological Pain of Gynecomastia

Gynecomastia affects men not just physically, causing pain, discomfort and breast swelling but more importantly, it affects them psychologically. 

Adolescent gynecomastia may resolve independently, but regression is unlikely in many others. When it does not regress spontaneously, its unfavourable appearance may lead to significant social and psychological impairment.

Patients who suffer from gynecomastia may:

  • Become so self-conscious that they avoid social activities, especially in environments requiring them to go topless, like swimming or chilling out by the pool or beach
  • Withdraw from mixed-gender gatherings due to embarrassment and anxiety
  • Refrain from intimacy, experiencing low libido or depression and mood swings that can affect relationships with partners

The embarrassment of having a feminised chest and the determination to get rid of it themselves is the primary reason why men with gynecomastia head to the gym to fix the issue.

Can Gynecomastia Go Away On Its Own?

Many assume gynecomastia always resolves on its own; but the percentage is rather low.

This misunderstanding frequently occurs because teenage boys can develop gynecomastia throughout puberty, which goes away in two years or less. In cases where gynecomastia persists for more than two years, it will likely become permanent.

Why Exercise Can't Reduce or Remove Gland Tissue

Three reasons working out doesn't fix gynecomastia:

1. Fat cells shrink with a calorie deficit; glandular tissue does not

Diet and exercise reduce body fat, but the firm gland tissue behind the nipple is a different type of tissue entirely and isn't affected by weight loss.

2. You can't spot-reduce fat, and you can't convert fat to muscle

When the body burns fat, it draws from stores across the entire body, not just the area being trained. 

Fat and muscle are also biologically distinct tissues; one cannot be "turned into" the other, no matter how strict the diet or training program.

3. Building chest muscle can make gynecomastia more visible, not less

Bigger pectoral muscles push outward, which can make the glandular mass more prominent rather than hiding it. 

Many patients who diet and train hard see their body weight drop while the breast tissue itself stays the same. If weight is lost quickly, they can end up with loose skin and stretch marks on top of the original problem.

Self-Check: Is It Glandular or Fat tissue?

Physically, you may notice breast soreness or discomfort; in some cases, discharge may occur.

Try a quick pinch and visual test to gauge the extent of the condition. If you feel a firm, fibrous mass when pinching the breast, that's likely gynecomastia.

An enlarged male breast is also highly visible when you're in a form-fitting shirt. In more severe cases, the breasts look like female breasts, with a degree of droopiness and loose skin present.

Gynecomastia Grades: Not All Cases Are the Same

Gynecomastia is classified into four grades based on severity:

Grade I

A small degree of breast enlargement in males, with localised tissue around the areola and a dark pigment area surrounding the nipple. Less noticeable with a top on.

Grade II (most common stage)

Moderate increase in breast size. At this stage, the tissue is already expanding sideways to the width of the male chest. Loose skin may be noticeable during physical activities or in tighter-fitting clothes.

Grade III

Moderate to significant enlargement of the breast, with more pronounced development of glandular tissue and fat, making the chest resemble a woman's breasts. Noticeable enlargement that may cause discomfort or embarrassment.

Grade IV

Substantial increase in breast size with a significant amount of excess skin and a more feminine breast appearance. Enlargement is severe, highly visible, and psychologically distressing.

Grade I Small enlargement; localised tissue around areola; dark pigment area; less noticeable with top on
Grade II Moderate increase; tissue expansion across chest width; loose skin noticeable during activity or tight clothing
Grade III Moderate–significant enlargement resembling women's breasts; noticeable discomfort or embarrassment
Grade IV Substantial enlargement with excess skin; highly visible; psychologically distressing

Gynecomastia vs Pseudogynecomastia: What's the Difference?

Pseudogynecomastia appears similar but they are entirely different. It is a distinctive condition, caused ONLY by fat deposits behind, around, and under the nipple, not glandular growth.

"Pseudogynecomastia is less commonly seen when compared to gynecomastia. For such patients, I will recommend weight management and lifestyle changes as the first-line treatment. When these methods don't work, liposuction is recommended," explains Dr Ivan Puah.

When Exercise CAN Help with gynecomastia

How to Know If Exercise Isn't Working

Consider surgical evaluation if:

  • You've lost significant weight, but the chest still looks prominent

  • You've trained consistently for more than 6 months with no chest improvement
  • Building chest muscle has made the problem more visible, not less

Surgical Options When Exercise Fails

Many patients experience depression, anxiety, and low self-esteem, and have already tried dieting, exercise, creams, fat freezing, and low-energy lasers without success before turning to surgery.

No. Push-ups can strengthen and tone pectoral muscles; however, they cannot spot-reduce chest fat or glandular tissue. When the body burns fat, it does so systemically throughout the body, not just in the area being exercised. Furthermore, glandular tissues will not respond to push-ups and will not be reduced or removed through this means. 

Can cardio or HIIT reduce man boobs?

Cardio and HIIT can reduce overall body fat, but they can't treat gynecomastia itself, since the condition is a distinct enlargement of glandular tissue rather than a simple result of being out of shape. In fact, many men find the condition appears more pronounced after weight loss because the fat around the gland shrinks first, making the gland itself more visible.

Does building chest muscle hide gynecomastia?

No, and it can backfire. Since gynecomastia is glandular tissue, building the pectoral muscles underneath can push the tissue further outward, making it more noticeable rather than less.
