Patient's Profile
Ms M, in her mid-30s, is a mother of two children.
Background
Ms M experienced significant changes in her breasts after breastfeeding her children. She noticed that her breasts became saggier and their size had decreased. Seeking professional advice to address her concerns, she consulted with plastic surgeons in Singapore, who recommended breast implants as a solution. However, Ms M was not enthusiastic about this treatment option and sought a second opinion with Dr Ivan Puah.
Consultation & Assessment
During the consultation, Ms M expressed concerns about losing her femininity as her breasts became flatter and droopier. She is self-conscious about the physical changes and would like to seek an alternative solution since she is not keen on breast implantation, as she dislikes foreign objects in her body.
Treatment Plan
Autologous fat grafting emerged as a viable option after a thorough discussion with Ms M. The procedure's fat retention rate, potential risks such as cyst formation, and the pros and cons of the surgery.
Ms M also expressed concerns about the armpit bulges as the axilla enlarged during breastfeeding. The bulges did not reduce in size. Upon examination, these bulges were axillary accessory breasts, which will require surgical removal. Ms M proceeded with fat grafting to the breasts and removal of axillary accessory breasts.
The surgeries were carried out smoothly in 2021 while the patient was under twilight sedation in an accredited Day Surgery Center:
ADR-C Breast Enhancement (Breast Fat Grafting)
Hip (donor site) liposuction is done to extract a substantial amount of high-quality fat, centrifuged and grafted to both breasts (recipient site) to increase volume, lift and improve the shape.
Axillary/accessory breast removal
Accessory breast removal surgery in both axillae.
Post-Op Recovery & Results
Ms M's recovery was uneventful, with no signs of hematoma or seroma. She raised concerns about the scarring of the axillary area and the hips, to which Dr Puah offered reassurance. He explained the scar discolouration would gradually improve over time and advised the patient to apply sunblock and scar gel whenever necessary. Patience is the key to the healing process.
Ms M's last review took place in 2022. At this point, the surgical scar discolouration had improved, and she expressed her satisfaction with the overall outcome of the procedures.
